It depends on whether it is a kit or adult and whether or not the skunk was properly de-wormed when it was young. If you acquired a skunk and do not know whether it is up to date on de-worming, be cautious when you do the first de-worming. Some Vets give less dosage in case there is a massive die off which can become impacted.
Below are the links to de-worm.

He was born last May & was dewormed at the pet store. They said to deworm again in November but just wanted to know going forward how often?

He should have been de-wormed 2 to 3 times depending on when he reached 4 lbs. That is when you start with Panacur liquid dosed by the Vet. By the time he is 5 lbs or more, you should be able to dose him using his current weight. Once he is adult size, you can buy Panacur C granules online for his weight. Because it doesn't appear that he was properly de-wormed as a kit, you have to pick up the schedule. I would need his current weight to say exactly where.
